This year, FirstFleet is helping our employees get “On the Road to Good Health” by offering major incentives for program participation. If you have not already created an account on BlueAccess, that is your first step! We will be sending you confirmation of your incentive eligibility using the BlueAccess system, and if you are not signed up, you could miss out on important information!
Remember: If you complete the following steps before August 1st, you will avoid paying a substantial premium differential in 2015 that will amount to significant savings for families and singles alike.
- Complete a biometric screening using the instructions below.
- Fill out your Personal Health Assessment (PHA) by logging into BlueAccess and choosing “Get Started”. If you need assistance setting up your BlueAccess account follow this link.
Biometric Screening Instructions
Complete your screening at an onsite event, or go the doctor of your choice—all you have to do is ask that provider to fill out a physician screening form (download here). You can find the form on the Company website under the Employee section.
