We wanted to provide one last opportunity for all participants in the medical plan to complete the wellness screenings this year.
The deadline for completing these assessments has been extended to AUGUST 31, 2016.

EVERY covered EMPLOYEE under our health insurance plan must complete BOTH the Personal Health Assessment (PHA) and a Biometric Screening
BEFORE August 31, 2016.
To complete the PHA, please log on to www.bcbst.com and go to Blue Access. Instructions to complete the PHA can be accessed from this link.
You can also take this assessment on the app called "AlwaysOn" too. Download the app from the Apple Store or Google Play. (This makes taking the assessment VERY EFFICIENT!!)
To complete the Biometric Screening, a basic annual well visit (physical) must be done by the employee’s medical provider or on-site at one of the Biometric Screening events that have been scheduled at local terminals. Biometric Screenings may also be done at many of the clinics that perform DOT physicals for FirstFleet drivers. Employees should check with their local manager about the availability of the Biometric Screenings at local clinics.
