FINAL REMINDER! DEPENDENT VERIFICATION DEADLINE IS NEXT FRIDAY APRIL 14, 2017

Don’t forget!  This Friday – April 14th is the deadline for HMS to receive the verification documents for all of your dependents on the Medical plan.  

If you need help identifying which documents you need to send in or need assistance locating the agencies in your community where you can obtain new birth certificates, marriage certificates or other legal documents,  please call HMS directly at (844) 845-8350.
